Mafia: The Old Country takes players back to the birthplace of the Mafia — Sicily in the early 1900s. Before the neon lights of Lost Heaven or Empire Bay, the Mafia was forged in blood, tradition, and loyalty in the rugged villages of Southern Italy.

You play as Enzo Favara, a young man who escapes forced labour in a sulphur mine and is taken in by the Torrisi crime family. Amidst olive groves, dusty roads, and medieval hilltop towns, you must rise through the ranks of the family, balancing loyalty, betrayal, and survival.

The game focuses on roots of honor (Omertà), family feuds, and the transition from old traditions to the modern underworld. Horse-drawn carriages give way to early automobiles, shotguns to tommy guns, and rural vendettas evolve into the foundation of global crime syndicates.
